Our High Risk Patient Programs are specifically designed for those experiencing one or more chronic medical conditions. Our programs provide the customized care, tools and resources needed to ensure patients stay healthy and independent while also helping to prevent complications and hospitalizations.

Alzheimer’s & Dementia

Our experience of providing care for seniors in their homes allows us to identify the many needs of those with memory loss illnesses such as Alzheimer’s or dementia. Our Alzheimer’s & Dementia Patient Program is specially designed to support and empower those living with the complexities of these conditions. Our program provides: 

Memory Care at Home. Experienced Philadelphia home health aides (HHAs) provide comprehensive, in-home assistance with activities of daily living. One of the tools our HHAs use to deliver effective and personalized care is a customized Philadelphia Memory Box, which promotes positive interaction and engagement.

Plan B: Relief Care for Unexpected Situations. Emergencies and unplanned situations call for a plan of action. Let us help you develop a go-to procedure so when the need arises, you have the peace of mind knowing your loved one is taken care of safely. 

Memory Café: A Mutual Support System. Connect with others in a safe, fun and interactive setting for early-stage memory loss participants, caregivers, family and friends.

In addition to our in-home Alzheimer’s patient program, we also own and operate Grand Oaks Assisted Living communities in Palm City and Jensen Beach that have state-of-the-art Memory Care Centers. 

Heart Failure Management

While heart failure cannot be cured, the condition can be successfully managed with our Heart Failure Patient Program, which promotes a heart-healthy lifestyle through self-help tools, weight management, exercise and more. A core component of our Heart Failure Management Program is treating common symptoms of heart failure, such as fatigue, swelling and shortness of breath.

Chronic Lung Conditions

Chronic lung conditions, including COPD, emphysema, and chronic bronchitis, bring many challenges for both the patient and their family. If left unmanaged, these conditions can be both physically and emotionally demanding, and their symptoms can adversely affect the patient’s quality of life. 

Our Chronic Lung Conditions Patient Program helps patients regulate their symptoms and significantly improve their overall wellbeing.

Living with diabetes often requires daily—even hourly— upkeep. If left uncontrolled, diabetes can lead to serious health problems, including heart disease, stroke, kidney disease, and blindness. Our Diabetes Management Patient Program is designed to educate and assist patients with self-care techniques to help control their condition and improve their quality of life. Stroke Management

Suffering a stroke can physically, mentally, and socially affect an individual and make it challenging for them to regain a sense of normalcy. Our Stroke Management: Patient Program utilizes a multidisciplinary approach that addresses all aspects of a patient’s life that has been affected by a stroke. Our program is specially designed to help patients adapt to new life abilities and reach a maximum level of independence.

Fall Prevention

Unintentional falls can be difficult to predict, and even more difficult to recover from. We offer a Fall Prevention Program that assists patients with specific challenges that may increase their susceptibility to a fall.
