SKILLED NURSING “RN & LPN”
Our in-home skilled nursing services assist patients who require long-term medical care or short-term rehabilitative services while recovering from a surgery or illness.
Our skilled nursing care services are administered by a Licensed Practical Nurse (LPN) or a Registered Nurse (RN) and include:
- Assistance with medical machinery, including ventilators
- Central and peripheral IV medication administration
- PICC line dressing changes
- Wound care
- Total Parental Nutrition (TPN) with G-tube or other feedings
- Catheter administration and maintenance
- Management of chronic health conditions, including diabetes and heart failure

Our nurses take a collaborative approach to in-home patient care, always involving the patient, their physicians, and their loved ones in the treatment process. Each skilled nursing patient receives a personalized, in-depth assessment of their current medical condition, overall health, and environment. This information is used to create an individualized care plan that is tailored to the patient’s unique physical and emotional needs.
Keeping patients and their families informed also helps patients manage their care better and helps them make healthy progress toward optimal wellness.
Skilled Nursing Care Along with Home Health Aides
Many families find that using personal and companionship care provided by Home Health Aides (HHAs) along with skilled nursing care is often the best choice. Once a patient returns home from having surgery, an LPN or RN can address their medical and rehabilitative needs while an HHA can help with bathing, meal preparation, light housekeeping, medication reminders and more.
